Prior to being processed by the school secretary ALL sections need completed:

• Work Permit Pre-Application Form

  • Student/Applicant Information

  • Signed and dated by parent

• Pledge of Employer

  • Signed and dated by employer

  • Employer's Tax ID required

• Physicians Certificate

  • NOT required if student has a current physical on file at the school

  • Signed and dated by physician
